Option B -$18.80, $81.70, or $143.55.   20% Discount for participation in Health Management Program. Premiums deducted before taxes.

Prescription drugs purchased at Medicine Box Pharmacy of Rutherfordton, employee co-pay 10% for generic drugs, with $2.00 minimum, 20% for preferred drugs and 30% for non preferred drugs. At other network retail pharmacies, employee co-pay 25% for generic with $7.50 minimum, 35% for preferred drugs and 45% for non-preferred drugs
